Important Information for Your Surgery Day

A current history and physical is required prior to surgery. The history and physical (H&P) will need to be completed by your Primary Care Physician and faxed to the Surgery Center at (904) 861-3599. This medical clearance can not be dated more than 30 days prior to your date of surgery. Anything older than 30 days, will require an update and additional clearance from your Primary Care Physician.

• Prior to your surgery, a nurse will be contacting you to conduct a pre operative interview. At this time, you will be asked a series of questions to ensure proper care is given. A nurse will give you information regarding your medications, arrival times, and informing you of anything you may need to know.
  
• You MUST bring this packet of information with you on your surgery day.
  
  
• You should have NO food or drink after midnight the night before your surgery. During your pre operative interview a nurse will give you instructions on any medications that you may need to take.
  
  
• Please arrive at your scheduled arrival time given. This will allow proper registration time and give the nurses ample time to prep you for your procedure. This is usually 45 minutes to 1 hour before your scheduled surgery time.
  
  
• Make sure to bring all insurance cards. A copy will be made upon your arrival to ensure that your claim is billed properly. Please bring any co-pay or fee, if instructed.
  
  
• Be sure to wear loose comfortable clothing on this day. You will be required to wear a button up shirt. It is recommended that you wear warm clothing and expect a chilly environment.
  
  
• Please do NOT wear any jewelry. All jewelry should be removed and left home prior to your surgery.
  
  
• No make up should be applied on this day.
  
  
• There are NO cell phones allowed at the Surgery Center. If you should need a phone, our receptionist will assist you in using the center's phone.
  
  
• All surgical patients are required to have reliable transportation to and from surgery. Family members are always welcome to stay in our waiting room during the procedure. Our waiting rooms are equipped with the latest technology to enhance your visit with us. This is a Wireless Internet hot spot.
